The Georgia Lions Lighthouse Foundation connects Georgians in need
with vision services.
SERVICE APPLICATION

Chamblee Clinic Hours:
Chamblee Clinic Glasses only Walk-in Hours:
Tuesday, Wednesday, Thursday 10am-1pm or from 2pm-3:30pm.
We are closed from 1pm-2pm for lunch and on Monday and Friday. Please bring your glasses prescription and your ID, no appointment needed.

The Georgia Lions Lighthouse Foundation works with a network of ophthalmologists to provide patients with optical surgery when needed.
We provide surgery assistance for:
- Cataracts
- Glaucoma
- Diabetes-related vision loss
Our mission is to serve Georgians who need us most. Please review the requirements and supporting documentation below and if you qualify, patients can apply for services either online or by printing, completing, and mailing an application.
Requirements for Services include:
- Resident of Georgia for at least one year
- Do not have insurance or other way to pay for surgery
- Meet the financial guidelines

The VSP Vision™ Eyes of Hope® gift certificate program operates nationally to provide access to eye care for adults and school-age children in need. VSP® Eyes of Hope gift certificates cover a comprehensive eye exam and, if prescribed, new glasses at a VSP network doctor’s office.
Georgia Lions Lighthouse Foundation is proud to partner with local Lions Clubs around the state to provide adults and children, who qualify, with a certificate voucher to receive essential vision care at no cost to the patient.

PAYMENTS
The Georgia Lions Lighthouse Foundation believes all patients should have access to vision care. As a result, fees are established based on the patient’s economic status.
